- MEDICAL & INFORMATION RELEASE FORM: Streamlined, Single-page release form collecting all signatures, T-Shirt order, etc.
- ONLINE REGISTRATION: Teachers register their students and pay online electronically via links below. This form is similar to the form used in the audition process.
- CREDIT CARD & CHECK: Just like in the audition process, credit card, eCheck, and physical checks are acceptable payment methods.
- DEADLINES: Medical release forms must be mailed and postmarked by October 6, 2017 and received by October 13th. Online registrations must be received by October 13th as well.
- COST: The participant cost for the 2017 All-State Choir is $75.00. This fee covers the conductor’s fee, music, equipment rentals, and insurance. This fee DOES NOT cover hotel accommodations or travel costs for All-State participants. (Optional T-Shirts are $15)
